Integrating Digital Coaching, Telehealth, and Behavioral Science to Improve CFRD Management

Cystic Fibrosis-Related Diabetes (CFRD) is a complex condition requiring both medical management and behavior-driven self-care strategies. Recognizing the gap in patient support, I designed and implemented a 12-week telehealth-based intervention that combined diabetes education, coaching, and digital health tracking to improve CFRD outcomes.

Program Overview

This pilot program leveraged telehealth and behavior-based coaching to help participants develop sustainable self-management skills while tracking progress in real time. The key components included:

  • Personalized health coaching & goal-setting to improve disease management

  • Digital health tracking using Dario Health glucometers and logging software

  • Expert-led weekly webinars featuring endocrinologists, dietitians, and pharmacists

  • Structured virtual group & one-on-one coaching for accountability and behavior change

  • Peer-driven support networks to reduce diabetes distress and enhance motivation

Measured Outcomes & Impact

  • Sustained A1C Reduction: 100% of participants maintained lower A1C levels at 1-year follow-up

  • Lower Diabetes Distress: Significant reductions in emotional burden and regimen-related stress

  • Scalable Telehealth Coaching Model: Demonstrated effectiveness of remote engagement and behavioral coaching

  • Improved Adherence & Patient Empowerment: Participants gained greater autonomy in disease management

Recognition & Presentation

Following the program’s successful outcomes, I was invited to present these findings at the 2020 North American Cystic Fibrosis Conference (NACFC). My presentation focused on the role of telehealth and integrative coaching in improving CFRD outcomes and demonstrated the potential for scalable, digital-first coaching models in chronic disease care.
